TOFF PLUS (Paracetamol, Dextromethorphan, Chlorpheniramine and Phenylephrine)

TOFF plus capsules contains the combination of the following: Paracetamol, a central analgesic which exhibits good and prompt antipyretic action and is chemically 4-hydroxyacetanilide; Chlorampheniramine maleate, an antihistamine that helps to control allergic symptoms and is chemically ϒ-(4-chlorophenyl)-N,N-dimethyl-2-pyridinepropanamine maleate; Dextromethorphan hydrobromide, an effective antitussive and is chemically (+)-3-Methoxy-17-methyl 9α, 13α, 14α-morphinan hydrobromide monohydrate; Phenylephrine Hydrochloride, a decongestant that promotes nasal drainage and relieves sinus pressure and is chemically (R)-3-Hydroxy-α-[(methylamino) methyl] benzenemathanol hydrochloride.

The drug is useful in conditions like fever associated with common cold, allergic rhinitis, nasal congestion, post nasal drip, headache

Rationale for combination

TOFF plus is a combination of an antihistaminic, antipyretic, α-adrenergic receptor agonist and an antitussive. The drug is useful in conditions like fever associated with common cold, allergic rhinitis, nasal congestion, post nasal drip, headache etc.

Paracetamol is useful in treating fever. Chlorampheniramine is useful in decreasing allergic symptoms of the disease. Dextromethorphan hydrobromide, is a cough suppressant and useful to suppress acute dry cough which is interfering with normal functioning of sleep. Phenylephrine Hydrochloride reduces nasal congestion. Hence TOFF plus is a rational combination of these drugs

Pharmacology

Paracetamol: It is a known antipyretic-analgesic that acts by inhibiting prostaglandin biosynthesis. It has some anti-inflammatory activitiy also

Chlorampheniramine maleate: An alkylamine, is a competitive antagonist of histamine at H1 – receptors. Chlorampheniramine maleate antagonizes various physiological and pathophysiological effects of histamine including increased capillary permeability and dilation, the formation of edema, the ‘flare’ and ‘itchy’ response and the constriction of gastrointestinal and respiratory smooth muscle.




Dextromethorphan: Have non opiate-mediated cough suppressant action. Further Dextromethorphan hydrobromide and its major metabolite dextrorphan does not have any opiate mediated analgesic action or respiratory depressant actions in animals. Animal studies suggest that the main site of action of Dextromethorphan is on the centrally induced cough response

Phenylephrine Hydrochloride: Is relatively selective α1 adrenoceptor agonist and thus have vasoconstrictor and decongestant properties.

Indications

TOFF plus capsules are anti-cold, decongestant, anti-allergic and anti-pyretic. Hence these should be used in common cold, fever associated with common cold, allergic rhinitis, nasal rhinorrhea, sneezing, nasal/sinus congestion and post nasal drip

Contraindications

Hypersensitivity to any of the active ingredients of the drug or other sympathomimetic agents, severe hypertension, alcoholics and patients on monoamine oxidase inhibitors, antidepressants, anxiolytics, hypnotics and sedatives and tranquilizers.
