ARTMETHER INJECTION

Nomenclature

This drug is known as Haojian Zhusheye in Chinese Phonetic. Artmether Injection is its English name. The drug is mainly composed of Artmether.




General information:
Group: Antimalaria agent
Oily solution for injection 80 mg in 1-ml ampoule
Artmether is a lipid-soluble methyl ether of dihdroartemisinin which has very rapid schizontocidal activity against blood forms of P. falciparum and P.vivax. After intramuscular administration, peak plasma concentration are attained within about 6 hours.
Artmether has been reported to clear fever in severe falciparum malaria within 30-84 hours. Artmether injection is available as a colourless or clear oily solution of artmether in ampoule for intramuscular administration only.

Uses
Treatment of slide-confirmed severe falciparum malaria in areas where there is evidence that quinine is infective. Radical care should then be effected with a full dose of an effective oral antimalarial such as mefloquine.


Indication:
Treatment of malaria due to P.faciparum and the first aid of critical malaria or when resistance to other antimalarial is suspected

Dosage and administration:
Adults and children over 6 months: 3.2mg/kg as a loading dose by intramuscular injection, followed by 1.6 mg/kg daily until the patient is able to tolerate oral medication or for  a maximum of 7 days.
The initial dose for adult is 160mg, followed by 80 mg each time from the 2nd to the 5th day. The dose for the overweight patients should be increased on the basis of the individual weight or under the doctor’s prescription.

Children:
Five days with initial dose of 3.2mg/ kg on day one followed by 1.6mg/kg for the next 4 days.

Pregnancy:
Little experience has been gained with the use of this drug in pregnancy but it should be withheld if it is lifesaving to the mother. It should be used with caution in the first trimester of pregnancy since some fetus absorption has been observed

Adverse reactions:
Clinical dosage exhibits slight adverse reactions. A transient low fever and reticulocytopenia may occur in individual cases. Slight rise of SGOT and SGPT may occur in individual cases. Arrhythmia may occur in rare cases (such as ventricular tachycardia).
Neurotoxicity has been observed in animal studies but not in humans
Cardiotoxicity has been observed following administration of high doses of artmether

Drug interactions:
Studies and reviews in the literature demonstrated that the active substance of Artmether had no interactions with other drugs on decreasing therapeutic effects and increasing toxic and side effects in bodies

Storage:
The oily preparation should be stored in tightly closed containers, protected from light.
